Fantastic experience: Every time I have been to this dental clinic everyone is very helpful and kind. Never had a bad experience. I have receding gums and they are always gentle when cleaning my teeth.

Fantastic experience: My experience was great from the front office staff, the assistant, Dr and the hygienist. They made me feel right at home and the routine of my new patient visit flowed perfectly. Natalie was amazing and didn't judge me for needing so much work!

Fantastic experience: This was my first time at Forum Dental and it was a wonderful experience. All the way from the receptionist to the hygienist and doctor, everyone was very friendly but professional. My hygienist was Carissa and I haven't had such a great hygienist in many years and I'm 75 years old. I will always request her if I possibly can. If anyone asks where to go for dental work, I will recommend Forum Dental.

Fantastic experience: Our first experience here was great! It was my 3 & 5 year olds first visit to the dentist and they worked hard to make sure they were comfy and supported. The really impressive piece was that we had an issue with our insurance and brought the wrong cards showing that they were covered (totally my fault) but the lady at the front desk worked so hard to help and called our insurance right then and there to get it resolved before we even left. It was SO appreciated!! True kindness and advocacy goes such a long ways!!

Went in today to get my dentures relined 5/16/23 And I came home and was able to eat chips on both sides of my mouth it was wonderful thank you so much Forum Dental

Positive experience: The staff and dentist were extremely nice and very helpful. I was treated very well. However the lab they use for bridge work is very low quality. I had asked not once but twice for certain instructions on the way I wanted them to look. Bridge work is very expensive. A person should have a say on what they want. The first bridge was straight across not much curve a large gap between the teeth where food would have stuck there. They had to sand and grind off and on for 2 hours just to get them to fit. I refused them and was very disappointed they also didn’t make the two front teeth longer than the eye teeth. Second time another 3 weeks wait. This time they were quite a bit darker and the eye teeth were longer than the front ones. I commented about a vampire. I was not pleased at all. They sanded down the eye teeth some and there was nothing they could do about the color. I was tired of dealing with all of it. So I have the bridge in. But not that happy about it and probably won’t be smiling as much now. As I said before the dentist and staff were excellent a 5 star rating.

Negative experience: Forum Dental sucks,took them about 8 months to make 1 plate and then they tried to scam me out of $500 to make the other plate. Do not go there.

Fantastic experience: I am not a fan of needing dental work but my appointment with Forum Dental on Dec. 3 in Lebanon, MO was exceptionally good. I I did not wait and all the staff were friendly and compassionate. The work was done efficiently and gently. I actually look forward to my future appointments here.

Fantastic experience: Carissa and Dr. Osborne are wonderful, kind, and compassionate. I’m not so easy to work with due to my gagging reflex and they were both very careful with that difficulty, as well as very understanding and sweet about making me feel welcome and my needs cared for.

Fantastic experience: I had an excellent experience at my recent dental visit. My hygienist, Olga, was absolutely the best. She was incredibly pleasant, kind, and made sure I was comfortable throughout the entire appointment. She took her time with the cleaning and never made me feel like she was rushing to finish. It was truly the most thorough and gentle dental cleaning I’ve had in a very long time. I’m extremely happy with the care I received.

Positive experience: Appointment was moved from original date over a month later then originally date. Only issue with my appointment was it took the dentist over 20 minutes to arrive to perform her checks after the dental hygienist was done.

Negative experience: Love the place! Staff is great, experience is always good. Totally recommend you calling makjng an appointment! Edited: 2025. Dr osborn extracted a tooth for an implant and had me pay $800 cash for a bone graph. Then sent me to springfield, oms for the implant. They said i wasn’t a good candidate for an implant at that location and acted super surprised to find out my dentist had already done the in unnecessary procedure. Told me to go get a bridge! Thanks Dr. Osborn, guess your like all the rest. Ill never be back, dont trust you and I’ve already started to tell my story to anyone that asks. Awful experience.

Negative experience: Appt. was 7/3/25 .. had the yearly cleaning done, unfortunately there was two cavities found .. OK let's fix them .. uh .. was told next appointment available .. was .. " May 2026 " yes I asked to repeat it because I thought I heard it wrong .. Come on .. be better than that. Right !!

Positive experience: I have received excellent customer service from Front Desk staff, Dentist & my Hygienist, Ashley are terrific in providing services, information & insight into better oral care. Obviously others must also agree, as appointment scheduling can be somewhat challenging.

Fantastic experience: The office staff is helpful and courteous. My dentist Vashti Osborn is spectacular. The dental hygienist that gave my anesthesia shot was perfect. Always good appointments and even when I forgot my earrings at one appointment they still had them 3 months later. I hope they always take my dental insurance as I do not want to find another office.
